Applicants

It is our aim to get to know all prosective students through our application process for admissions.  Application requirements include a completed application form, one letter of recommendation, 1 personal essay, and official transcripts from high school and any colleges attended.  Please review the brochure for additional information.

Must be certified in BLS for Healthcare Providers.  A copy of your certification must be included with your application requirements.

Tuition is $6,300 for the twenty-four month program, not including the cost of textbooks.

Admissions

Applicants who have officially been accepted to the Mercy Hospital School of Radiologic Technology's program need to complete, sign, and date:

The Health Evaluation Form (completed by your physician) and the Personal Health Information Form (completed by the student) need to be delivered to the employee health nurse at the time of the personal health interview.  The Immunization and Student Information Forms (both completed by the student) should be mailed to the School of Radiologic Technology at the address above.

Please make a copy of all completed forms and retain the copy for your records.

It is mandatory for all students who are receiving financial aid to complete an entrance loan interview.  Go to http://mapping-your-future.org and click on the On-Line Student Loan Counseling icon in the bottom right-hand corner of the home page.  Then choose the appropriate counseling type.

The above site meets all current U.S. Department of Education regulatory requirements.  Once completed, please print the document and retain for your records.

Current Students

To request official transcripts, medical information, or other personal information, please print the Release of Information Form, completed in its entirety, sign the form, and mail it to the address above.

The form may be filled out online and printed, or you may print the form and fill it out manually, after printing the form. Please remember to sign the form before submitting the form.
