Patient Information
On Your First Visit
You will be asked to complete a new patient questionnaire, which includes questions about your medical history and your insurance coverage. Please be sure to bring your insurance card with you so we can make a copy for our records. If you have medical records from a previous provider, please bring them with you or request that they be forwarded to our office.
Payment Policy
In most cases, we can bill your insurance company directly. You will be responsible for any co-pay your insurer requires. Co-payment is expected at the time of service. Financial assistance is available. If you have questions about your bill please contact patient accounts at (207) 879-3470 or by email.
Insurance
We take Medicare and Medicaid assignment and participate in most major insurance programs and managed care plans.
Special Needs
All office locations are accessible. Mercy also provides sign and spoken language interpreting services. These services are provided at no charge to the individual. Please ask for assistance when you schedule an appointment.
