If you are a DO, you will need to contact The Board of Osteopathic Licensure (State House Station 142, Augusta, ME 04333) at 207-287-2480. You can also download an application form from their website.You should ask for an Application to Practice Osteopathic Medicine.The cost is currently $350, which you will need to pay yourself. Depending on your contract terms and choices with us, we may reimburse you for these expenses when you arrive in July and provide us with an accounting.
