Hospice Care

Who and What is Home Hospice
Think of VNA Hospice as a resource - a varied team of health care professionals who work with you to develop and provide a comprehensive, individualized plan of care. We'll involve you and your family in the direct care of your loved one - and we'll be there to offer support and respite when needed.  You may stay in your own home while receiving hospice care.

Our hospice team is available on call 24 hours a day, 7 days a week. With 24-hour pharmacy access, we coordinate the provision of pain medication, assist with symptom control, and help you access equipment and supplies, including beds, wheelchairs and other medical equipment.

The VNA Hospice Team includes:
The client and his or her support system (family, paid caregivers, neighbors, friends).
Bereavement Support
Home Health Aides
Nurses
Social Workers
Spiritual Support
Hospice-trained Volunteers
Physical and Speech Therapists
Nutritionist

Home Hospice Admission
VNA Home Health & Hospice accepts referrals from physicians, hospital discharge planners and social workers as well as directly from patients and families.

The following criteria are required for admission:

  • The patient has a limited life expectancy of less than six months confirmed by the patient's primary physician.
  • The patient and family consent to hospice care
  • The patient and family agree to the treatment plan based on hospice philosophy of providing care and comfort only, rather than cure.
  • Identification of a primary care giver.
